Authors present current standards of surgical treatment of Crohn's disease. Special stress is laid on still existing difficulties in proper assessment of indications for surgery as well as the most proper time to perform it (timing of surgery). The paper assembles the most common surgical strategies (resection, strictoplasty, by-pass anastomosis, diverting ileostomy, techniques used in treatment of perianal fistulas) with relation to possible location of disease. As pathological changes in Crohn's disease may be located in virtually every part of digestive tract and the course of the disease is unique and unpredictable in each patient, the surgical approach to Crohn's disease is far more complicated than in case of ulcerative colitis. Authors conclude that in majority of cases it is still impossible to establish any strict algorithms of surgical approach to Crohn's disease.
