The prevalence of obesity has reached global epidemic proportions. This is significant problem in both industrialized and developing countries. However there is still remained unsettled which measure of excess adiposity best predict cardiovascular risk. Obesity is associated with many clinical consequences and leads to progress of numerous comorbidities. Investigators proved its independent influence on appearance of arteriosclerosis, coronary heart disease, premature non-ST-segment elevation myocardial infarction, structural and functional cardiac disorders. Obese individuals have higher risk of left atrial enlargement even irrespective of arterial pressure and higher risk of atrial fibrillation. Paradoxically, although obesity is powerful risk factor for heart failure, patients with higher body mass index have a more favorable prognosis than lean heart failure patient. This „obesity paradox„ is observed also in several obesity-related cardiovascular conditions - hypertension, coronary artery disease and peripheral arterial disease. It was esta- blished that loss of weight may reverse the adverse effect of excess adiposity on cardiovascular risk progression. Present paper reviews recent literatu- re communications regarding the clinical aspects of overweight and obesity.
