The authors define myocardial viability and present mechanisms of loss of contractile function with preserved viability (stunning and hibernation). The most common clinical scenarios leading to stunning and hibernation are also discussed. Moreover, the authors present imaging modalities used in everyday practice for assessment of myocardial viability - echocardiographic techniques (parts of resting examination, stress tests, echocardiographic evaluation of coronary flow and coronary flow reserve), radionuclide techniques (single-photon emission computed tomography and positron emission tomography) and late-enhancement magnetic resonance imaging. The feasibility and diagnostic value of these modalities, as well as their limitations are also discussed.
