Antiplatelet and anticoagulant drugs are used for the prevention and treatment of cardiovascular disorders. Acute upper gastrointestinal bleeding (AUGIB) is a severe complication in patients receiving long-term oral antithrombotic drugs. Combined antiplatelet and anticoagulant therapy is associated with a high risk of gastrointestinal bleeding beyond that associated with each drug used alone. The use of proton pump inhibitor should be a standard treatment for patients who are at risk for peptic ulcer. This paper presents the clinical management of gastrointestinal haemorrhage complicating antiplatelet and anticoagulant therapy, as well as modification of antiplatelet and anticoagulant treatment in patients undergoing endoscopic procedures.
